Use to track belief conviction ratings across sessions, providing a visual representation of cognitive change over time. Start tracking once target beliefs have been identified, typically from session 3-4 onward.
Explain the tracking purpose: 'I'd like us to keep track of how strongly you believe certain thoughts over the course of therapy. At the start of each session, you'll rate how much you believe each one right now. Over time, we'll be able to see how these beliefs shift, which helps us know if what we're doing is working.'
For clients who find numerical ratings difficult, use visual analogue scales or simple low-medium-high categories. Track both the old negative belief and the emerging alternative belief to show the crossover point. For clients who become discouraged by slow progress, show the trend line rather than individual data points.
Avoid tracking so many beliefs that the exercise becomes burdensome. Limit to 2-4 key beliefs. Do not use the ratings punitively if beliefs are not shifting; instead, use the data to inform formulation revision and strategy adjustment.
The most informative pattern is when belief ratings plateau, as this often signals a need to change approach or address a deeper maintaining factor. Look for discrepancies between head and heart ratings, where intellectual belief change outpaces emotional belief change. Use visual graphs in session to make progress tangible and reinforce hope.
