When you notice a shift in your mood, complete each column working left to right. Start with the situation, then identify the emotion and hot thought. Gather evidence for and against the hot thought before constructing a more balanced perspective.
Use when the client has mastered basic thought identification and is ready for fuller cognitive restructuring. The 7-column format adds evidence gathering and balanced thinking, making it the gold standard for cognitive work in depression. Best introduced mid-treatment once the therapeutic relationship is solid.
Build from the 5-column record: 'You've been doing well at catching your thoughts. Now let's add a step where we look at the evidence for and against those thoughts, like a scientist testing a hypothesis. This helps us arrive at a more balanced perspective.'
For perfectionistic clients, emphasise that the goal is not to find the 'right' answer but to broaden perspective. For clients with concentration difficulties, complete collaboratively in session before assigning as homework. Consider digital formats for clients who dislike handwriting.
Not suitable if the client cannot yet identify automatic thoughts or differentiate thoughts from emotions. Avoid if the client uses it to intellectualise without emotional processing, or if repeatedly completing records without mood shift suggests the intervention needs re-evaluating.
The re-rating of emotion is crucial; if mood ratings never shift, explore whether the client is engaging with the evidence or just going through the motions. Hot cognitions identified in column 3 should connect to the formulation. Use Socratic questioning in session to model the evidence-gathering process before expecting the client to do it alone.
