A thought record is a structured tool from Cognitive Behavioural Therapy that helps clients identify and evaluate negative automatic thoughts. Originally developed by Aaron Beck as the Dysfunctional Thought Record, the 7-column version guides clients through a step-by-step process of cognitive restructuring.
The seven columns are: situation, emotions (with intensity ratings), hot thought, belief rating, evidence for, evidence against, and balanced thought with a re-rated belief.
